The table below shows Microsoft Word versions of our Pen99 Authorised contact forms.

We are currently accepting these forms by Egress secure email from a senior manager on behalf of the employing authority.

This form is for temporary use where it is not possible to print and complete the PDF version of the form with original signatures.

Employer completes to notify us of changes to employer details and authorised contacts for mail shot updates, pensions liaison contact, electronic users, actuarial valuation contact and senior manager.

Please ensure individual email contacts are included for each electronic user. The form must be either signed or emailed to the Pensions Communications team by a senior manager for your employing authority. 

Third Party Pen99

This form is for temporary use where it is not possible to print and complete the PDF version of the form with original signatures.

The third party payroll or pension provider, on behalf of the employer, completes to notify us of changes to third party details and authorised electronic users.

Please ensure individual email contacts are included for each electronic user. The form must be either signed or emailed to the Pensions Communications team by a senior manager from your third party organisation.
